Rehab on the Move - Occupational Therapy

Rehab on the Move Occupational Therapists are trained to assess and treat a variety of conditions that affect your ability “to do things”, in the comfort of your own home.

Occupational Therapists are often referred to by our clients as “the therapist that gets you doing things again”. This could include showering and dressing, signing your name, holding a hand of cards, doing the shopping, getting back to bowls, driving or catching a bus. Rehab on the Move’s Occupational Therapists are skilled at assessing your physical, cognitive and perceptual skills that may be limiting you in the performance of some of your daily activities, as well as having great skills at looking at your environment and making suggestions that would make it easier to be independent such as raising the height of your toilet or installing a rail in the shower recess. Rehab on the Move Occupational Therapists have a wide range of knowledge of electric scooters, wheelchairs and seating systems as well as many smaller aids that assist people to be more independent. Our therapists are able to trial equipment with you before purchasing to ensure it is going to achieve what it is intended to.

Who We See

Our experienced Occupational Therapists specialise in providing the following therapy services:

  • Hand therapy and splint making - stroke, acquired brain injury, traumatic hand injury, arthritis, spinal cord injuries
  • Cognitive and perceptual therapy - stroke, acquired brain injury, early dementia
  • Retraining in activities of daily living - stroke, acquired brain injury, spinal cord injuries, amputations, hip and knee replacements
  • Home modification advice
  • Wheelchair Prescription
  • Manual handling training for carers
  • Case management services for clients with complex needs

Assessment
At your first visit your Occupational Therapist will assess you in order to identify your strengths and weaknesses. Your Occupational Therapist will use a combination of standardised assessment tools as well as observing your functional performance of activities.

Your Occupational Therapist will record baseline data on your abilities at this initial assessment in order to be able to track change as you progress with your Occupational Therapy program.

Establishing Your Goals
Following your Occupational Therapist assessment you and your Occupational Therapist together, will determine your rehab goals. It is important that you are working towards goals that are important to you!

Your Individual Occupational Therapy Program

An individual Occupational Therapy program will be devised for you to work towards achieving your rehab goals. Your program will be specific to your problems and may be hand therapy, cognitive and perceptual retraining, retraining in the performance of daily activities.
Rehab on the Move Occupational Therapists travel with a range of equipment to ensure that all your Occupational Therapy needs are catered for at home.
If particular equipment is required to assist you such as a shower stool or wheelchair, your Occupational Therapist can arrange for you to trial the equipment before purchase.
With your consent your Occupational Therapist will liaise with your other health professionals to ensure a coordinated team approach to your recovery.

Evaluating Your Occupational Therapy Program
At each session your Occupational Therapist will be taking note of your progress and will upgrade your program as improvement is made. In the event that progress anticipated is not being achieved your Occupational Therapist will review your program and make changes to try and get the right results, it is no use persevering with something if improvement is not being seen.
Your progress will continue to be monitored until your goals are reached.

Case Conferencing
A very special part of your behind the scenes treatment is Rehab on the Move's case conferencing. This is an opportunity for your Occupational Therapist to discuss with other team members your program and progress. This provides the opportunity for your therapist to liaise with other highly qualified professionals to ensure the highest standard of treatment.
Rehab on the Move team includes Physiotherapists, Occupational Therapists, Speech Pathologists and great Administration Staff.
