Rehab on the Move's team of Occupational Therapists, Physiotherapists and Speech Pathologists accept referrals to conduct Initial Needs Assessments from:
- Compulsory Third Party Insurers
- Life Time Care and Support Scheme
- Workers Compensation Insurers
- Income Protection Insurers
- Estate Managers
- Hospital Discharge Planners
- Department of Ageing Disability and Home Care
- Care Providers
Assessment
Our therapists will assess your client's functional, physical and communication status as relevant using standardised assessment batteries as well as skilled observation of the performance of functional tasks by your client.
Rehab Plan
Following the assessment our therapists will prepare a report outlining the findings of the assessment and devise a Rehab Plan for your client. The Rehab Plan will outline the recommended treatment for your client to achieve functional improvement that will impact positively on their quality of life.
Our therapists are trained to provide thorough clinical justification for the need for therapy services, equipment or care requirements.
Case Management and Treatment
Rehab on the Move is able to follow through with the provision of case management and treatment services recommended within the Initial Needs Report. All recommendations will be discussed with the referring agent and costs approved prior to the commencement of treatment or case management services.
